Een zorg die bleef hangen in de onderlaag van een organisatie. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Toen in 2018 en 2019 twee Boeing 737 MAX-toestellen kort na het opstijgen neerstortten, wezen de vingers al snel naar een technisch mankement: MCAS, software die de neus van het toestel automatisch omlaag duwde op basis van data van \u00e9\u00e9n enkele sensor. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Maar wie de interne documenten las, zag dat het probleem veel eerder was gesignaleerd. De zorgen waren er. De kennis was er. Alleen: de top kreeg het niet te horen \u2013 of wilde het niet horen. Dat is geen uitzondering. Het is het patroon.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Bij GKN Fokker Aerospace begon het net zo: een ogenschijnlijk kleine kwaliteitsafwijking groeide uit tot een complex probleem door tegenstrijdige data en versnipperde analyses.
