This is one of the most expensive habits in an organizational environment – not because the action is bad, but because action aimed at the wrong problem leads to recurring issues, wasted time, team effort, and decisions that don’t hold up over time.

What Happens When You Solve the Wrong Problem?

You see it in meetings all the time. A problem is raised, and within minutes the conversation shifts to actions, fixes, and next steps. It feels productive. It feels efficient.

But solutions applied without proper diagnosis almost always treat symptoms, not causes. The result is a problem that keeps coming back.

This pattern shows up in predictable ways:

Recurring issues that never fully resolve, no matter how many times the team addresses them

Rework that quietly drains resources without appearing on anyone’s radar

Hard-working teams that aren’t making real progress because their effort is misdirected

Confidence in decisions begins to erode.

The organization isn’t slow. It’s misdirected and aimed at the wrong target.

What is Root Cause Analysis?

Root cause analysis is the practice of asking why a problem exists — multiple times — to truly understand what is wrong and before deciding how to fix it. The goal is to identify the underlying cause of an issue, not just its visible symptoms.

One of the core principles of the Toyota Production System is the “Five Whys.” Before acting, ask why multiple times, repeating until you reach the true source of the performance deviation. The goal is not more analysis, rather it’s better decisions. It takes a few extra minutes upfront and saves significant rework downstream.

Why Do Organizations Skip Root Cause Analysis?

Speed pressure is the most common reason. Diagnosing a problem thoroughly feels slow, especially when there’s urgency to act. But as Harvard Business Review has noted, many organizational failures aren’t caused by a lack of action — they stem from solving the WRONG problem extremely well. Skipping diagnosis doesn’t save time. It borrows it, at high interest.

Why Does This Matter More in the Age of AI?

AI can generate answers instantly—but it cannot ensure you’re solving the right problem.

If the question is off, the answer will be too. Fast answers amplify bad assumptions just as easily as good ones. That makes structured thinking and clear problem definition more important, not less.

The advantage is no longer just speed. It’s direction.

How Can Teams Improve Problem Definition?

Start by slowing down just enough to ask better questions:

What problem are we actually trying to solve?

What evidence do we have?

What assumptions are we making?

Have we separated symptoms from causes?

What happens if we solve this—but the issue still returns?

This doesn’t require more time, rather more discipline.

What Happens When Organizations Get Disciplined About Root Cause Thinking?

They move faster over time, not slower.

They spend less time revisiting the same issues.

They make fewer false starts.

They build confidence in their decisions.

The Bottom Line

In a world that rewards speed, the instinct is to act quickly. But the real advantage comes from pausing long enough to define the problem clearly before moving forward. Organizations that build this discipline into how they work spend less time on recurring fixes and more time making progress toward solving problems.

Where do you see this most in your organization: jumping to solutions, or investing time to understand the real problem?

Most teams jump to solutions before they fully understand the problem.
